The Swede’s manager has revealed that world number 14 has been suffering depth perception in his left eye.
The 39-year-old has not reproduced the kind of form this season that saw him win twice on the European Tour in 2008, which included the Mercedes-Benz Championship and Alfred Dunhill Links Championship.
It was due to ill health – an inner ear infection – that Karlsson pulled out of the Wales Open, and the big Swede has only managed one top-10 finish in eight starts this season.
Karlsson, who finished tied for fourth in last year's US Open at Torrey Pines, has been replaced by American Andrew Svoboda for the showdown at Bethpage Black.
